Checklist

Completed application forms have to be postmarked by February 15.

• Three (3) completed recommendations
  (*Please note that there must be at least one from a teacher and from a work supervisor, the third can be of your choosing. Dr. Craig and Mrs. Hernandez CANNOT complete a recommendation for you.)
• DPD Verification Statement (must be an original). Those who have not completed their degree should file a completed "Declaration of Intent to Complete Degree and/or CADE Minimum Academic Requirements" form.
• Transcript(s) of all college class work attempted, including graduate school (must be an original).
• A current resume.
• A letter stating:   
  1 -  Why you chose Andrews University to do your Dietetic Internship
  2 -  Your career goals
  3 -  Your immediate plans after the Internship
• $45.00* check for application fee (to Andrews University).
• Completed application form.
• Completed Dietetic Internship Additional Information Requested form.
