Download the floor hockey team registration form and save it to your computer. Fill out the team registration form and then e-mail it to djardine@andrews.edu.

The team registration form is due by 5 PM the day of the Captain's meeting scheduled for 2.14.11 at 7:30 PM in the Johnson Gymnasium balcony. A team representative must be present at the captain's meeting.

Don't have a team? All "free agents" will be assigned to a team but must be present at the Captain's meeting.
