IRB Application Form
Effective January 1, 2013 all Principal and Co-Investigators must submit an IRB NIH Online Training fwith the application materials. Applications without certifcates will not be reviewed. Please click on the link below to complete your NIH Training.
Please download the Word or PDF document below, complete all the sections and submit the application form along with other documents as an email attachment to irb@andrews.edu, or fax a copy to the attention of IRB at (269) 471-6543 or mail to: Institutional Review Board, Andrews University, Berrien Springs, MI 49104-0355
For students please note that we also require a signature or an email approval from your advisor that your IRB application materials have been reviewed and approved before submission to IRB. Student IRB applications without the advisor's approval will not be processed.
Please submit the application materials electronically in one document in PDF or Word fprmat as an email attachment to irb@andrews.edu. The package should include the applcation form, research protocol, data collection tools, participant informed consent form (as applicable), institutional consent letter or amd IRB (as applicable), recruitment materials, NIH certicates for the PI and Co- investigators and the advisor, non english translations (as applicable). All applications that do not follow these guidelines will not be reviewed. They will be returned to the applicants.
Please note: Any content typed in the Word document can be saved, however content typed in the PDF document cannot be saved, the form must be printed out.
