Student Success and Wellness Choices are coordinated by the Counseling and Testing and Student Success Departments. If you would like to plan one of these Choices, please review our objectives and suggested program formats. You may download a Program Proposal Form and submit it to the Student Life Office for review and approval. Proposals should be submitted at least four weeks before the program date. You will be notified within a few days of submission as to approval.
