THE GIFT OF LIFE DONOR REGISTRY

The Gift of Life Donor Registry is a means of making your wishes known to
your family.
Other means of expressing your wishes are:

to include donor information on the driver's licence
to record your wishes in your Living Will
to employ a Durable Power of Attorney for Health Care

However, this information is not always immediately available. Therefore, the
Gift of Life Registry is a practical means to ensure that your donation wishes,
at the time of death,
are known.

How the Registry Works

When you return the attached Donor Form, your decision and personal
information will be added to the computer system at the Gift of Life Agency.
This information will be available to your hospital and family at the time of your
death, when your family will be asked to grant permission for anatomical
donations.You are advised to also share your intention with your family N O W.

Body for Anatomical Study

This donor registry does not include the donation of your body for anatomical
study. This must be done with the medical school of your choice.
