1Informed Consent

 

Topic and Purpose This survey is part of a research project studying the attitudes of millennials toward history as it relates to women in ministry in the Seventh-day Adventist church and how the Library can meet the needs of millennials with regard to SDA history topics.  

Duration of Participation
The survey will take most participants about 5 minutes to complete. It consists of six questions plus some demographic questions.  

Participant Risks
The risks to survey participants are minimal. If you are uncomfortable answering any question, you may skip it. You may withdraw from the survey by closing your web browser window and emailing the researcher with a request to have your responses removed from the study.  

Voluntary Participation Participation in this study is voluntary. Refusal to participate will involve no penalty or loss of benefits to which you may otherwise be entitled.  

Privacy/Confidentiality
The survey is completely anonymous. No identifying information will be requested. Participants will only be asked for gender, approximate age, religious affiliation, and classification such as student, faculty or community member.
