2015 Open Enrollment

We are pleased to announce the upcoming 2015 open enrollment period for our benefit programs for all benefit eligible employees. Open enrollment is the employee's annual opportunity to elect and/or make changes to certain benefits for the new plan year, effective July 1, 2015. Please review the open enrollment information provided below.

OPEN ENROLLMENT PERIOD
April 1–16

PREMIUM DISCOUNT
This year, to receive a wellness discount on health premiums effective 7/1/15, employee (and spouse, if applicable) only need to fulfill one requirement:  meet the 2015/2016 biometric health standards with current biometrics (see wellness program).

COMPLETING OPEN ENROLLMENT
To make open enrollment changes, you may self-enroll by logging in with your Andrews username and password to the online benefit management system (bswift) at www.andrews.edu/go/mybenefits.

  • You may review your personal information under MyProfile and any necessary documents under Library
  • To begin enrollment, click "Enroll Now"
  • Be sure to save each benefit, even if no changes are made
  • Once enrollment is complete, review your elections and confirmation statement
  • Be sure to have an electronic record saved by selecting to email your confirmation

If you have any issues in bswift (with login/error pages) please contact Chris Dalian (bswift representative) at (616) 419-2961. For enrollment or benefit questions, please email benefits@andrews.edu or schedule an appointment with a benefit counselor. 

Please remember:

  • Flexible Spending Account (FSA) elections do not passively roll into the new benefit year (IRS regs)
  • You must re-enroll in the flex accounts every year
  • FSA changes effective 7/1/15 must be made during open enrollment through bswift

2015 Wellness Program for Premium Discount

Who is eligible to participate?
All employees and spouses covered under the Andrews University medical benefit plan or are eligible for coverage. Both employee and spouse covered under the medical benefit must complete the program requirements to earn the discount.

What are the program requirements?
This year, to receive a wellness discount on health premiums, the only requirement for employee (and spouse, if applicable) is to meet the 2015/2016 biometric health standards with current biometrics*. If standards are not met, alternate satisfaction activities must be completed.

*Are my biometrics current?
You may be required to submit new biometric values.  Lakeland will send an email to your Andrews email address, notifying you (and your spouse, if applicable) whether new biometric screening is required or not. If you (and your spouse, if applicable) are not required to submit new biometric values, you have met the health standards and qualify for the wellness discount.  If you have questions about the email, your biometric information, or if you do not receive one by March 17, please send an email to LakelandCare_aHealthyu@lakelandhealth.org.

Please note: the HR Office does not have information regarding employee biometrics so please contact Lakeland Care for any questions regarding the email received.

HEALTH TARGETS

  • Three health screenings being measured:  blood pressure, glucose, and BMI (Health Standards)
  • For any actionable/urgent value, in order to qualify for the reward, the Alternative Satisfaction Activity (ASA) must be completed
  • ASA for actionable value: participate in & complete myHealtheir Lifestyles
  • ASA for urgent value: participate in & complete myHealtheir Lifestyles AND meet with a Health Coach to develop a plan to address your health risk (review plan with your doctor) 
  • Please refer to benefit booklets, handouts, and website for further details

HEALTH SCREENING
For those required to submit new biometric values, the onsite biometric health screenings will be held on March 24 & 25 from 6:30–11 a.m @ Dining Services. The screenings are available only to employees and spouses who have been contacted by Lakeland for new screenings (other dependents are not eligible).

  • Requires a 12-hour fast (water only, no coffee or juice)
  • You may take your medications as prescribed
  • You are encouraged to drink water to remain hydrated
  • A light breakfast will be available to screening participants

In the email sent by Lakeland Care, a link to the scheduler is provided, should you be required to submit new biometrics.

  • Sign-up must be made by 5pm the day before the screening
  • An email confirmation will be sent after appointment set up (check spam or junk folder)
  • A reminder notice (email/text) will be sent

Lakeland Care Health Coaches will be onsite to review biometric screening results on April 7 & 8 from 8:00 a.m.– 5:00 p.m. (further information to be provided at screening appointment).

Please note: the Office of Human Resources does not have information regarding employee biometrics.  Please contact Lakeland Care for any questions regarding biometrics at LakelandCare_aHealthyu@lakelandhealth.org

ALTERNATIVE TO SCREENING
A Primary Care Provider (PCP) wellness visit verification form may be submitted to Lakeland Care. This form may be completed based on biometrics from an office visit within the last 12 months. This physician form is due by April 17.
