No chromosomal abnormalities
Philadelphia chromosome (Ph1)
Autoimmune disorders may be seen
BCR-ABL translocation
May transform to:
Variable course
Results in activated tyrosine kinase production
Large-cell lymphoma (Richter syndrome)
Blast phase (L2-like blasts)
May be confused with:
Small-cell lymphoma
t(9;22)-22 goes to 9
Ph1 positive = 2% in AML
Ph1 positive = 5% in childhood ALL
Ph1 positive = 20% in adult ALL
Ph1 negative CML= CMmL
Chronic phase:
Lasts 3 years
Accelerated phase:
Fails to respond to therapy
Blast crisis phase:
>20% blasts+promyelocytes in bm and pb
1/3 to ALL & 2/3 to AML
