Work Permit
Safe Sleep & Shaken Baby Training (Follow directions on link)
Health and Safety Training (follow directions)
Preventing and Controlling Disease, and Immunizations (updated Bloodborne Pathogens Class) (required $15 class. You will be reimbursed and paid for your hours when the courses are completed and the certificates are submitted to the office)
Copy of TB Test Results (You have to have this test to go to Andrews. Should be in your medical records. If not, you will have to get a new test done.)
Copy of First Aid training certificate (will be offered at The Crayon Box this fall)
Copy of pediatric, child, and adult cardiopulmonary resuscitation (CPR) training certificate (will be offered at The Crayon Box this fall)
Staff Information Card
Professional Character Clearance
Policy on Abuse and Neglect (for your information)
Michigan Child Care Background Check Form
Fingerprinting (once paperwork is completed, The Crayon Box office sends you information so you can set up an appointment to be fingerprinted. You must then go to the appointment in Benton Harbor. Must be done before starting work )
Do you know how to recognize the signs of daycare abuse? (required reading)
Remind App (required)
Crayon Box Parent Handbook (required reading)
Summer Camp Parent Handbook (required reading if working for Summer Camp)
