Supervisor's Accident Report form



When an employee is injured on the job, the injury must be reported to Human Resources on a Supervisor's Accident Request form. Further information may be found on the Worker's Compensation page. In order to open and print the Supervisor's Accident Report form, you will need Adobe Acrobat Reader. If you do not have Acrobat installed on your computer, you can download it from the Adobe website .
