Your child's immunization records (Vaccines Required for Childcare and Preschool in Michigan)
Recent physical from your pediatrician
$50 per child for registration fee
Completed registration forms
Your child's schedule and start date

An advanced payment for two weeks must be made by the first day of attendance.
