Basic Life Insurance

Eligibility:
All regular full-time University employees working a minimum 35 hours per week.

Who Pays:
University.

Effective Date:
Starts on the first of the month following the date of eligibility.

Description:
The purpose of this benefit is to provide financial assistance in meeting the employee's share of the expenses of the final illness and funeral as well as the needs of the survivors.

Includes an accelerated benefit in the event the covered person is diagnosed as terminally ill and both a conversion and a portability privilege in the event the covered person loses coverage.

Benefit Provisions: (effective 07/01/2005)
Employee $ 100,000  
Spouse $ 50,000  
Child $ 10,000  (for each child until age 24*)
Stillborn $ 750

No enrollment is necessary. However, you must complete a Beneficiary Designation Form and place it on file with your employer. If you have not completed a Beneficiary Form or need to update your designations, please see the Benefits Coordinator.

If the spouse or dependents are also serving as employees of the University, only one benefit per death will be made. Age-related reduction in spouse's coverage is stated in the Group Life Plan Document.

Please be advised that:
  1. Andrews University will continue to pay the full premium for the life insurance.
  2. Because the amount of coverage is in excess of $50,000, the IRS requires that it be included in income, using the IRS Premium Table, and be subject to Social Security and Medicare taxes.
  3. Due to the tax implications, you have the choice of waiving the higher benefit.
  4. You must sign a waiver release by May 31, 2005 to decline the higher benefit. If you are married, the waiver must also be signed by your spouse.
  5. If you waive your right to the higher benefit, you will not have the opportunity to choose the higher benefit at a later date.
Employees on a voluntary basis may supplement their Survivors Benefit amounts by participating in a North American Division of Seventh-day Adventists Group Supplemental Life Insurance plan, or Accidental Death & Dismemberment plan, for themselves, their spouses, and dependent children through payroll deductions for the costs thereof.
