Payroll Forms
The following forms are provided here for your convenience. Once completed, they can be submitted to the Payroll department. If you have any questions concerning a form or need submission information, please contact us at payroll@andrews.edu or 471. 3325.

Use only when no time manager is available. Please submit to payroll by 1pm on Monday of payweek. - Application for Long-Term Sick

This form stops payroll deductions to employee/student accounts and non-ARP retirement plans. (Subject to approval.) - Direct Deposit/Payroll Debit Card Form
Paychecks can be deposited into any bank in the US if an account number and bank routing number are provided. - Government Forms
