Community Living
This form must be filled out by all single undergraduate students under the age of 22 years of age who will be living with a parent, grandparent, the brother or sister of a parent, or older sibling (22+) in the community (45-mile radius). Students and host relatives must come to the Student Life Office to sign the form. Application to live in the community must be made annually.
Student Organizations
This form is to be used to gain approval from the Student Life Office each time a student organization plans a campus event or a daily off-campus event. Overnight trips require a different form and approval process set up by the Univesity's Trips and Tours Committee. Please contact the Student Life Office for details.
This form is used to either register a new organization or re-register an existing organization. All student groups must be registered as an organization in order to function on campus. Three officers and a full-time faculty or staff advisor are required to start an organization. Student organizations must register annually.
This form is used to set up an Interdepartmental Charge (IDC) acccount with the University accounting office. An IDC account allows you to deposit funds, make cash and check requests, and charge at some on-campus locations such as the Transportation Department and Lithotech. Once an account is set up, it does not have to be renewed annually.
This form is used to charge student organization fees to students' accounts. This must be done within the first two weeks of each semester.
